LCDMHA Registration Info

Please Note:

• Payments can be made via e-transfers to treasurer@lakecowichanminorhockey.com Password: HOCKEY
• Family Discount: (U11 - U18) 2^nd child is $50 off; 3⁺ children is $25 off each child. (For discounts to apply, all children must remain within LCDMHA.  No discounts on Initiation or Novice registration).
• NON REFUNDABLE Rep Try-out Fee: $100 per child.  Must be paid by June 1, 2021
• Players not paid in full prior to season start up will not be able to participate with the team until we have received full funds.
• There will be no refunds beyond 3 weeks of the start up date.
• Completed Kidsport or Jumpstart applications if applicable. Applications are available at http://kidsportcowichan.com/apply/ or Canadian Tire JumpStart at phone number 250-748-7529 or Fax 250-748-0054.
